Four of the five patients were seriously ill, the Arizona Daily Star reported Wednesday. But in one case, an otherwise healthy 73-year-old man died after surgery because a nurse accidentally swapped his oxygen and suction tubes, the newspaper said.

Problems documented at the hospital are enough to threaten its state license and Medicare certification.
