ANNA, Ill. -- Haskell "Sally" Maddox, 75, of Anna, died Thursday, Feb. 22, 1996, at her home.

She was born March 6, 1920, in Pulaski County, daughter of Albert and Sadie Wiard Goodman. She and Jack P. Maddox were married May 14, 1938, in Jackson, Mo.

Maddox was a retired nurse, and member of First Baptist Church in Jonesboro.

Survivors include her husband, and a daughter, Carolyn Crooks of San Diego, Calif.

She was preceded in death by a brother and sister.

Friends may call at Lutz and Rendleman Funeral Home in Anna from 6-8 p.m. today.

Funeral service will be at 1:30 p.m. Saturday at the funeral home, with the Rev. Jerry Ford officiating. Burial will be in Dongola American Legion Cemetery.
