SHORT-TERM OUTCOMES: CAPE COUNTY TRANSIT AUTHORITY RECEIVES ADDITIONAL SUPPORT FROM GOVERNMENT ENTITIES TO COORDINATE EXISTING TRANSPORTATION SYSTEM.

INTERMEDIATE OUTCOMES: TRANSPORTATION FOR EMPLOYMENT IS AVAILABLE 24 HOURS A DAY, SEVEN DAYS A WEEK; TRANSPORTATION FOR BASIC NEEDS AND QUALITY OF LIFE SERVICES IS AVAILABLE FROM 8 A.M. TO 6 P.M. MONDAY THROUGH SATURDAY.

LONG-TERM OUTCOMES: RESIDENTS HAVE ACCESSIBLE, AFFORDABLE AND RELIABLE TRANSPORTATION.

(PRIMARY CARE, DENTAL, MENTAL HEALTH)

SHORT-TERM OUTCOMES: RESIDENTS ARE AWARE OF THE IMPORTANCE AND AVAILABILITY OF PRIMARY-CARE PROVIDERS AND PERSONAL RESPONSIBILITY; COLLABORATION AMONG HEALTH-CARE PROVIDER TO IMPROVE SERVICES; CHILDREN HAVE ORAL SCREENINGS BY AGE 3 AND SEALANT TREATMENTS BY AGE 6; DENTAL TREATMENT CARE IS AVAILABLE; ADULTS ARE AWARE OF THE IMPORTANCE OF ORAL HYGIENE AND THE EXISTING TREATMENT OPTIONS; FAMILIES AT RISK OF THEIR CHILDREN DEVELOPING MENTAL HEALTH ISSUES ARE IDENTIFIED; COMMUNITY'S APPROACH TO MENTAL HEALTH CARE IS EVALUATED FOR IMPROVEMENT.

INTERMEDIATE OUTCOMES: RESIDENTS HAVE ACCESS TO PRIMARY CARE PROVIDERS; RESIDENTS HAVE ACCESS TO DENTAL HEALTH CARE; MODEL FOR PREVENTING OR MINIMIZING IMPACT OF EMOTIONAL DISTURBANCE AND MENTAL ILLNESS IS DEVELOPED; COMMUNITY'S EXISTING DELIVERY SYSTEM OF SERVICES IS EFFECTIVE AND ACCESSIBLE.

LONG-TERM OUTCOMES: RESIDENTS ARE AWARE AND HAVE ACCESS TO THE MOST AFFORDABLE AND HIGHEST QUALITY MEDICAL SERVICES AVAILABLE; RESIDENTS HAVE GOOD DENTAL HEALTH; FAMILIES HAVE EMOTIONAL STABILITY OR ACCESS TO AVAILABLE SUPPORTS; MENTAL HEALTH CARE SYSTEM IS COORDINATE, COMPREHENSIVE AND ACCESSIBLE.

Alcohol and Drug Abuse

(ADULTS AND YOUTH)

SHORT-TERM OUTCOMES: ADULTS WITH CHILDREN IN THEIR CARE ARE AWARE OF AND HAVE ACCESS TO INTERVENTION AND TREATMENT SERVICES; EFFECTIVE PREVENTION, INTERVENTON AND TREATMENT STRATEGIES ARE PROMOTED FOR PRESCHOOL AGE THROUGH COLLEGE AGE YOUTH.

INTERMEDIATE OUTCOMES: ADULTS WITH CHILDREN REDUCE THEIR ILLEGAL OR ABUSIVE USE OF ALCOHOL AND DRUGS; YOUTH HAVE ACCESS TO CONTINUUM OF PREVENTION, INTERVENTION AND TREATMENT PROGRAMS FROM PRESCHOOL TO COLLEGE.

LONG-TERM OUTCOMES: ADULTS DO NOT ENGAGE IN ILLEGAL OR ABUSIVE USE OF ALCOHOL AND DRUGS; YOUTH DO NOT ENGAGE IN ILLEGAL OR ABUSIVE USE OF ALCOHOL AND DRUGS.

Family Issues

(POVERTY, HOUSING, PARENTING SKILLS, FAMILY VIOLENCE/CHILD ABUSE)

SHORT-TERM OUTCOMES: FAMILIES ARE AWARE OF AND HAVE ACCESS TO EXISTING SUPPORT SYSTEMS; SYSTEM OF WRAP-AROUND SERVICE IS AVAILABLE TO MEET MULTIPLE NEEDS OF FAMILIES; FAMILIES HAVE THE OPPORTUNITY TO PARTICIPATE IN PARENTING EDUCATION PROGRAMS AND FAMILY-BASED ACTIVITIES; DEVELOP A COMMUNITY INITIATIVE AROUND DEVELOPMENTAL ASSETS.

INTERMEDIATE OUTCOMES: IMPROVED COORDINATION OF SUPPORT SERVICES; FAMILIES WILL PARTICIPATE IN PARENTING EDUCATION PROGRAMS AND FAMILY BASED ACTIVITIES; DEVELOPMENT ASSET COMMUNITY INITIATIVE WILL BE IMPLEMENTED.

LONG-TERM OUTCOMES: CHILDREN AND FAMILIES LIVE IN A SAFE ENVIRONMENT; CHILDREN ARE MORE SUCCESSFUL; FAMILIES ARE MORE SELF-SUFFICIENT.
